Focused Evaluation of Foot, Heel, and Ankle Pain in Midtown Manhattan
Foot and ankle pain can interfere with walking, stairs, commuting, exercise, and athletic performance. The challenge is determining which structure is responsible for the symptoms.
Pain in the heel, Achilles tendon, ankle, arch, or forefoot may arise from a tendon, ligament, joint, bursa, plantar fascia, bone, or peripheral nerve. More than one structure may contribute to the same clinical problem.

Foot and Ankle Pain Is Not One Diagnosis
The foot and ankle contain multiple closely connected joints, tendons, ligaments, and nerves. Pain location alone does not always establish the diagnosis.
For example, posterior heel pain may involve the Achilles tendon, its insertion on the heel bone, the retrocalcaneal bursa, or surrounding structures. Lateral ankle pain after a sprain may persist because of ligament injury, joint irritation, tendon pathology, or nerve involvement.
The goal of the evaluation is not simply to label the complaint as “foot pain” or “ankle pain.” It is to determine which structures are most likely contributing and whether further imaging or targeted treatment is appropriate.
Conditions Commonly Evaluated
Evaluation may be appropriate for selected patients with:
Achilles tendinopathy or suspected partial tendon injury
Retrocalcaneal bursitis
Plantar fascia–related heel pain
Acute or chronic ankle sprains
Peroneal tendon pain or suspected tendon subluxation
Posterior tibial tendon dysfunction or pain
Ankle or foot osteoarthritis
Joint effusion or synovitis
Morton neuroma or interdigital nerve irritation
Tarsal tunnel syndrome
Sural, tibial, superficial peroneal, or deep peroneal nerve irritation
Repetitive-use and sports-related foot or ankle injuries
Persistent pain despite physical therapy, bracing, or prior treatment
This list is not exhaustive. Some conditions require orthopedic, podiatric, rheumatologic, neurologic, or surgical evaluation rather than treatment within the practice.
A Diagnosis-First Foot and Ankle Evaluation
Patients often seek evaluation because pain has persisted despite rest, physical therapy, footwear changes, bracing, or a prior diagnosis that does not fully explain the symptoms.
A focused visit may include assessment of:
The exact location and pattern of pain
The mechanism of injury and progression of symptoms
Walking and load tolerance
Ankle and foot range of motion
Tendon strength and resisted movement
Ligament stability
Joint-specific findings
Swelling, effusion, or focal tenderness
Peripheral nerve irritation or entrapment
Lower-extremity mechanics and contributing restrictions
Prior X-rays, MRI studies, or other imaging may also be reviewed when available.
The objective is to develop a working diagnosis during the clinical evaluation and determine what additional information, if any, is needed to make a treatment decision.
The Role of Diagnostic Ultrasound
Diagnostic ultrasound can be particularly useful for selected foot and ankle complaints because tendons, superficial ligaments, bursae, fluid collections, and peripheral nerves can often be evaluated dynamically.
Ultrasound may help assess structures such as the:
Achilles tendon
Peroneal tendons
Posterior tibial tendon
Plantar fascia
Retrocalcaneal bursa
Ankle joint
Selected ankle ligaments
Superficial peripheral nerves
Dynamic examination may also be useful when symptoms occur during movement or when tendon subluxation or mechanical irritation is suspected.
Ultrasound is not automatically the correct study for every foot or ankle complaint. X-rays may be more useful when arthritis, alignment, or bone injury is suspected. MRI or CT may be appropriate when evaluating selected osteochondral injuries, occult bone pathology, deeper structures, or more complex ligament and tendon injuries.
Imaging is selected based on the suspected structure and the clinical question being asked. Current radiology appropriateness criteria similarly distinguish among radiography, ultrasound, MRI, and CT based on the suspected source of foot or ankle pain.
Conservative-First, But Not Passive
A conservative-first approach does not mean repeatedly waiting while the same symptoms continue.
Treatment depends on the structure involved, the severity of the problem, functional limitations, prior care, and the patient's goals.
Management may include:
Activity and load modification
Focused physical therapy or rehabilitation
Taping, bracing, or selected orthotic strategies
Changes in footwear or training mechanics
Osteopathic evaluation and osteopathic manipulative treatment (OMT) when regional mechanics may be contributing
Targeted treatment of associated muscle or soft-tissue dysfunction
Image-guided injection when a specific joint, bursa, or other structure is an appropriate target
Selected peripheral nerve blocks
Advanced injection options for carefully selected tendon, ligament, or joint conditions
Referral for orthopedic or foot and ankle surgical evaluation when appropriate
The goal is to match treatment to the suspected pain generator rather than move every patient through the same sequence of therapies.
When Pain May Come From Outside the Foot or Ankle
Not every symptom felt in the foot or ankle begins there.
Nerve pain may arise from a peripheral nerve in the lower leg or ankle. In other patients, symptoms extending into the foot may be influenced by the lumbar spine or a more proximal nerve pathway.
Hip pain, knee pain, and gait mechanics may also alter how load is transferred through the ankle and foot.
A broader examination may be appropriate when the location of the symptoms and the local findings do not fully match.
Sports Medicine and Repetitive-Use Injuries
From a sports medicine perspective, foot and ankle injuries are common in running, court sports, martial arts, soccer, weight training, and other activities involving repetitive loading or rapid changes in direction.
Returning to activity depends on more than whether pain has temporarily improved.
Tendon loading capacity, ligament stability, strength, range of motion, and the demands of the specific activity may all be relevant. For active patients, the objective is to identify the injured structure and develop a realistic plan for rehabilitation and return to training.
When Surgical Evaluation May Be Appropriate
Some foot and ankle conditions are better treated surgically.
A significant tendon rupture, unstable injury, advanced structural degeneration, progressive deformity, or other mechanical problem may require evaluation by a foot and ankle surgeon.
Non-surgical evaluation should not be used to delay appropriate surgical care. In selected patients who are likely to need surgery, treatment and rehabilitation before surgery may still help address modifiable mobility, strength, and regional mechanical factors as part of broader preoperative planning.

Foot and ankle pain is often treated according to symptom location alone: heel pain receives one protocol, ankle pain another.

Frequently Asked Questions
Do I need an MRI for foot or ankle pain?
Not necessarily. The appropriate imaging study depends on the suspected structure and the clinical question. X-rays, diagnostic ultrasound, MRI, and CT provide different information. Imaging should be selected based on what needs to be evaluated rather than ordered reflexively.
Can diagnostic ultrasound evaluate Achilles tendon pain?
Ultrasound may be useful for evaluating selected Achilles tendon problems, including tendon thickening, structural changes, partial injury, and surrounding fluid or bursal abnormalities. The examination and clinical history remain important, and MRI may still be appropriate in selected cases.
Can ankle pain continue after a sprain has healed?
Symptoms may persist after an ankle sprain because of residual ligament injury, instability, joint pathology, tendon dysfunction, scar-related changes, or nerve irritation. A focused evaluation can help determine whether the original injury fully explains the ongoing symptoms.
Can foot pain come from a nerve?
Yes. Peripheral nerves in the lower leg, ankle, and foot may become irritated or entrapped. Symptoms extending into the foot may also sometimes be influenced by a more proximal nerve pathway. The distribution of symptoms and examination findings help guide the evaluation.
Do you treat sports-related foot and ankle injuries?
Selected sports and repetitive-use injuries are evaluated within the practice. Treatment depends on the suspected diagnosis and severity of the injury. Conditions requiring surgical management or specialized foot reconstruction are referred appropriately.
